Hi Jvalant,

"Leave of absence" is the term used. You can say, while on leave so-and-so will care for my responsibilities. Or you can say "in my absence" but the term leave of absence is what you are applying for.

As for an application, you will have to follow the guidelines laid out by your company policy. Some have an application other will just expect the request in writing.

When following proper business letter style, the salutation should be formal. Usually it starts with "Dear" and includes the last name of the recipient and most often a title. The name is followed by a colon.

thanks for providing a leave mail format. but in above mail format u wrote "would like to request a five day leave of absence for" ..the words leave and absence are used simultaneously. Is it used this way..? I just want to ask You, please guide me. Can we make this sentence easy?

And one more thing that I want is a leave application in permission-approval format.

In this a "grant me leave" sentence will be there.

Your reasons for leave of absence would probably fall under the Family and Medical Leave Act of 1993. In such a case, an employee must give 30 days notice prior to taking leave when foreseeable and the notice must include information specifying that the leave falls under Family and medical Leave Act. Be sure to include dates and the duration of the leave in your letter. Family and Medical Leave Act leave can be taken all at one time (12 weeks); one week at a time, one day at a time; on an intermittent basis; or on a reduced schedule of usual hours.

Check the details of the Family and Medical Leave Act of 1993 to see if you do qualify, then write your employer letting them know your plans including your willingness to work weekends if necessary.

i have to take CNA classes so my disabled brother doesn't loose his social security. it's only a 3 week absence and would work weekends if allowed. how do i write this?

Hi Isad,

If the company you work for has a personnel handbook check it for official company policies. However, if the company is small and doesn't have written policy, organize your thoughts before you put them in letter form. Ask yourself how long you want your maternity leave to be? And even beyond that, when it's time to go back to work, do you want to come back full-time, or perhaps you want to request permission to return as a part-timer for a period of time and then move back to full time? Do you have any paid time coming to you? Can things like sick days, vacation time, personal days, disability and any paid or unpaid time be used as part of your leave? You'll want to include any information like this in your letter.

In your case with so few people, I'm not sure what your boss will do to fill in your position. Offer suggestions like hiring from a temp agency, giving more hours to other employees, or whatever else you can come up with that works for your specific cir...stance.
