Sorry Misty, but I can't find a template for such small labels. I was wondering if you meant 1/2 x 1 3/4 inch labels. If so Template S-10432 is designed for those.

On the IRS website, PDF files are provided for all forms and can be viewed and saved using Adobe Reader software. XML/SGML files are provided for instructions and many publications. If you have paid a site for this form, you should be able to go to that site and download it and save it on your computer and print it out.
